HMPCA Comprehensive Report on Aden Governorate (8 July 2015)

Conflict in Aden governorate:

Heavy conflict persists with devastating consequences for trapped civilians. Aden governorate is divided into eight districts: Crater (especially Sierra), Al-Mu’alla, At Twahi, Khormaksar, Al-Mansoura, Sheikh Othman, Dar Saad, and Al-Buraiqeh. Basic utilities have collapsed in most if not all districts in Aden. Electricity is completely unavailable in Crater, Al-Mu’alla, At Twahi, and Khormaksar, which have been most affected by this conflict in terms of destruction, targeting, and systematic siege; and the majority of local residents there have displaced to the districts of  Sheikh Othman, Dar Saad, Al-Buraiqeh (or Brega) and Al-Mansoura, which houses the majority of IDPs (Internally displaced personals) in Inma city (now most population dense area in the governorate). And no one is safe anywhere in Aden, as these districts have recently been heavily shelled by the Houthi armed groups and Pro-Saleh forces, including Enma. 

Statistics on causalities and death (26March-1July 2015): 6307 and 644, respectively. Note, that these numbers do not included the cases that present to MSF (Doctors Without Borders) or to the health facilities that communications with has been lost.

Health situation in Aden governorate:

The health system in the province is indeed facing imminent collapse. Numerous critical health facilities have been attacked, forcing them to shut down. Local health workers continue to be targeted and with salaries (or an incentives) remaining non-existent; as such,  their level of commitment has reduced, resulting severe shortages of health staff across all active facilities.

Outbreak in Fibril Illnesses:

Malaria, mainly P. falciparum class (the more aggressive of the classes) and Dengue fever is now endemic in Aden governorate, which was not the case prior to this conflict (began on March 2015). Many health facilities lack the tool needed for their accurate diagnosis with fever being an overlapping syndrome. In effect, their mistreatment has been numerous and a serious issue.

According to Dr. Nasr Haidara, Director of Epidemiological Surveillance in Aden, from April-June of 2015, approximately 1786 (with 37 deaths) cases of febrile illnesses has been reported across health facilities in Aden. He notes that this is an underestimate as many are purchasing drugs from the black market and self treating at home. Furthermore, not all facilities keep track of the cases that present.

The International Medical Corps (IMC), along with other stakeholders, have been supporting all efforts to combat the dramatic increase in cases of febrile illnesses. This has included: vector control measures–insecticide spraying (which has been effective)–and promoting public health awareness. This campaign began on 10 June 2015 and has been executed under the supervision of Dr. Abdulnasser Alwali, Chairman of Higher Medical Public Committee-Aden (HMPCA). The preventative intervention campaign, however, has been hindered by insecurity and interruption in fuel supply. Furthermore, transport vehicles have been damaged (intentionally) by the opposing party and many have been confiscated; this has been reported to officials in Sana’a on numerous occasions, as of yet, we have only received promises for their return .

While Khormaksar, Crater, Al Malla, and Twahi districts have been difficult to reach, due to insecurity, they are the most endemic areas for febrile illnesses with the highest cases of death being reported (especially in Crater):

Major Constraints:

• Insecurity: health staff and volunteers are constantly targeted and threatened 
• Aden governorate remains under siege and the needed supplies are not able to reach us in a timely manner
• No financial support or budget, as banks in Aden remain shutdown from the central bank in Sana’a 
• Not enough transport vehicles and interrupted fuels supplies
• Internet connection and telecommunication networks are frequently disrupted

HMPCA Flash Update (7/7/15): Update on outbreaks in Aden governorate

July 7, 2015

From 27 June to 5 July 2015, we report 70 new cases (41 males, 25 females, 4 children) of acute fever secondary to infectious disease (i.e. malaria, Dengue fever, and unknown) and 0 deaths. This brings the total number of cases to 795, including 13 deaths.

Note:
-These statistics are an underestimate as they do not include the security comprised areas, where acess to health services remains severely constrained.

تقرير عن حملة الرش الضبابي لمواجهة تفشي حالات الملاريا وحمى الضنك عدن /يونيو ٢٠١٥م

تتكون محافظة عدن من ثمان مديريات (صيرة ,المعلا, التواهي,خورمكسر,المنصورة ,الشيخ عثمان ,دار سعد والبريقة) حيث شهدت ومازالت اشتباكات ومعارك شملت معظم المديريات , وتعاني المحافظة من وضع معقد جدا , حيث انهارت معظم الخدمات ان لم تكن كلها في ظل انقطاع كامل للكهرباء في أربع مديريات (صيرة ,المعلا, التواهي وخورمكسر)وهي المديريات التي شهدت دماراً شديداًمما أدى إلى نزوح شبه كامل للسكان إلى مديريات (المنصورة ,الشيخ عثمان , دار سعد والبريقة) وتعاني المحافظة من انعدام الخدمات الصحية حيث تعرض معظم المرافق الصحية للقصف وتم الإعتداء على الطواقم الطبية وعدم استلام الموظفين للمرتبات منذ اشهر وهو ما يحول دون ممارسة الكثير من العاملين في القطاع الصحي لأعمالهم مما أدى إلى نقص شديد في الطواقم الطبية في المرافق الصحية التي تستقبل المرضى مما عكس نفسه على مستوى الخدمة الصحية المقدمة.

وتعتبر محافظة عدن من المحافظات غير المبؤة بالملاريا رغم وجود معدل إصابة يختلف بحسب المواسم ونتيجة لحركة النزوح سوا ًء من داخل المحافظة أو من المحافظات المجاورة وهو ما جعل المحافظة تتعرض لتفشي جائحة الملاريا والأمراض الوبائية الأخرى

وتعد الملاريا المنجلية (P. falciparum) هي النوع السائد في المحافظة وهي من الأنواع التي تسبب الملاريا الوخيمة احتمال ظهور مضاعفات ناتجة عنها كبير جداً

ونتيجة لإنتشارحالات الحميات في المحافظة وتسجيل الكثير من الحالات في المرافق الصحية في محافظة عدن والمحافظات القريبة منها . ونتيجة لتزايد الشكوى من وجود حالات ملاريا وحالات حميات يشتبه بكونها حمى الضنك وبعد ان كنا قد قمنا بالنزول الى المرافق الصحية في المديريات التالية (المنصورة , الشيخ عثمان ودارسعد) وتم تزويدها بعلاجات الملاريا

ونتيجة لزيادة حالات الحميات ورغبة من الهيئة الطبية الدولية (IMC/International Medical Corps) ممثلة بالدكتورة دعاء كتبي عمر والدكتور وائل الحاج للقيام بحملة رش ضبابي للمحافظة يهدف الى تحريك المجتمع حول وجود وباء لحميات أحدى أسبابها حمى الضنك وحالات ملاريا مع إحتمال أسباب أخرى للحميات , كما أن عملية الرش سيكون لها أثر ايجابي عند السكان لما تمثله من اشعار الناس ان هناك اجراءات قد تم اتخاذها من قبل القائمين على العمل الصحي وتم وضع خطة للرش الضبابي بناء على الإمكانات المتاحة حيث عقدت عدة لقاءات تحت اشراف اللجنة الطبية الشعبية برئاسة أ.د.عبدالناصر الوالي رئيس اللجنة وبحضور

القطاعات ذات الصلة كصندوق النظافة الحاج قائد راشد والمؤسسة المحلية للمياه والصرف الصحي الأستاذ نجيب محمد أحمد والمهندس زكي حداد وصحة البيئة الأستاذ محمد موسى ومنظمات المجتمع المدني , حيث تكفلت السلطة المحلية بتوفير المشتقات النفطية اللأزمة لإنجاح الحملة .

وترافق إجراء الحملة القيام بدورات تثقيف صحي لعدد من المتطوعات والمتطوعين بهدف نشر الوعي حول الأمراض المنقولة بواسطة البعوض( الملاريا وحمى الضنك ) وكذلك بؤر تكاثر البعوض وحملة رفع مخلفات القمامة وإصلاح شبكة الصرف الصحي.

HMPCA Flash Update (30 June 2015)

We had a meeting (weekly) with the health staff and directors of health facilities (hospitals, health centers, and polyclinics) in Aden, Lahj, Abyan, and Al Dhale’e where we discussed the following:

• the health situation in the governorates and the possible solutions to overcoming the main challenges together, as a team
• the possibility of International Organization for Migration (IOM) support health facilities in the governorates by procuring a constant supply of water and to repair the water system in Al-Sadaqa Hospital (Sheikh Othman, Aden) and 22 May Hospital (Al-Mansoura, Aden) in Aden; and if possible to procure water storage tanks as a solution to their constant shortage of water supply (which IOM representatives in Aden acknowledge, including Ms. Salamah Mubarak, the Director of IOM office in Aden); Dr. Moheeb Ahweth, Aden Public Relief representative, has also agreed to help in resolving the water shortage across health facilities
• Establishing a training course for treatment of Dengue fever and malaria, which will be supported by Qatari Red Crescent, World Health Organization (WHO), and Ministry of Pubic Health and Population.

Also, we met with the health staff (who are currently working in Al-Sadaqa Hospital) of Basheeb Military Hospital (At Tawahi, Aden), including its director,  and we agreed to the following: to reactivate and support the facility with medicine and supplies and establish it as an inpatient facility (for patient recovery) for Al-Sadaqa Hospital and 22 May Hospital in order to relieve their load of patients, as both facilities are currently overwhelmed.

Lastly, we have created a department in Al-Sadaqa Hospital exclusively for patients with fibrille illnesses. Treatment will be free of charge thanks to the support of local organizations (“MAGD”).

تقرير إنجاز اللجنة الطبية الشعبية العليا- عدت 30 يونيو 2015
 تم عقد اللقاء الدوري الخاص بمدراء المستشفيات والمرافق الصحية والذي يعقد كل ثلاثاء ويظم ممثلي المستشفيات والمجمعات ومندوبي المحافظات عدن ولحج وأبين والظالع وقد تم النقاش حول :

• الوضع الصحي في المحافظات وابرز الصعوبات وتحدثنا عن سبل تذليل الصعاب بشكل تكاتفي وجماعي.

•  إمكانية دعم منظمة الهجرة الدولية للمستشفيات والمجمعات الصحية في المحافظات المعنية بالماء بشكل منتظم وعن إصلاح منظومة المياة في مستشفى الصداقه وم. ٢٢ مايو. مع إمكانية توفير خزانات مياه إضافية لحل مشكلة المياة اللتي تواجه هذه المراكز، والذي اشارت له الاستاذة سلامة مبارك مدير مكتب منظمة الهجرة الدولية في عدن.

• تأكيد ائتلاف عدن للإغاثة الشعبية في المشاركة لحل مشكلة المياة وتوفيرها للمرافق الصحية والايوائيه، والذي أشار له د. مهيب عوض ممثل الائتلاف.

• تحديد موعد البدء بورشة العمل الخاصة بعلاج ومكافحة حمى الظنك والملاريا يوم السبت والاحد 4,5 يوليو  2015 في مستشفى الصداقه على ان يرشح كل مركز صحي اربعه من الاطباء والممرضين لحضور الورشة التي ستقام بالتنسيق بين اللجنة الطبية الشعبية العليا - عدن ومكتب الصحة العامة والسكان وبدعم من منظمة الصحة العالمية والهلال الاحمر القطري.

تم عقد لقاء مع قيادة مستشفى باصهيب الميداني المتمركز حاليا في مستشفى الصداقه وتم الاتفاق على التالي:

• دعم المستشفى بالادويه والأجهزة الطبية.

• استقبال الحالات الجراحيه لمصابي الحرب بعد العمليات من مستشفى ٢٢ مايو لتخفيف الضغط على م. ٢٢ مايو.

• كما تم اليوم البدء باستقبال مرضى الحميات( الظنك والملاريا ) في قسم الحميات في م. الصداقه المدعوم من اتحاد المجموعات الجنوبيه( مجد) بالمجان.

• تم التنسيق مع مكتب الصحة العامة والسكان عدن بان يتم مخاطبة جميع الجهات الداعمة بان اي دعم دوائي يجب ان يسلم الى المخازن المركزيه ويجرد ومن ثم يوزع بإشراف الاستاذ د. عبدالقادر الباكري رئيس الهيئة العليا للأدوية نائب رئيس اللجنة الطبيه الشعبية العليا- عدن  والدكتورة سعاد ميسري مسؤولة المخازن المركزية اقليم عدن.

HMPCA Flash Update (7/1/15): 6307 casualties and 644 deaths in Aden Governorate (March 26th-July 1st, 2015)

On 1 July 2015, clashes in Aden governorate has resulted in 133 causalities and 27 deaths. This brings the total causalities and deaths (from 26 March 2015) to 6307 and 644, respectively.

Note, these statistics do not include:

• the cases reported by MSF facilities (Doctors Without Borders)
• the health facilities we have lost communications with
• the victims that could not reach any facilities at all
